If this illness will prevent you from attending TODAY's required activity for PCM I or PCM II, you must call or email (or both) the course coordinator immediately. With PCM, you can still complete this form even if you have not yet discussed this with your course director. However, you need to contact the coordinator LuAnne Stockton (firstname.lastname@example.org or 330.325.6776) immediately.
If you are going to miss ONLY PART of Infection and Immunity because of this absence, please say how many hours you WILL be there.
When you submit your absence report, a receipt of what you wrote will be automatically sent to your course director, the course coordinator and to you.
When you report an absence due to a religious holiday or a professional meeting, a receipt will also be sent to Dean Anita Pokorny.
